Issue is resolved. The problem was that the PowerEdge R710 had its boot drive (C:) pointing to an SD memory card in the box instead of the RAID disk. So even though the upgrades were being applied against the RAID disk, the system was still booting from the SD card (which has Dell's facttory installted ESXi 4.1). Changing the hard disk drive sequence fixed the issue.
